That's the vision becoming a reality in Jersey, thanks to a groundbreaking partnership between Graphnet Health and Family Nursing & Home Care (FNHC). This initiative, a landmark project, is backed by the Impact Jersey CareTech Challenge program and spearheaded by Digital Jersey. FNHC will be leveraging Graphnet's Remote Monitoring solution, powered by Luscii, to keep a close watch on the health of vulnerable islanders, whether they're at home or in community settings. The project has received an impressive CareTech grant of almost £800,000, specifically earmarked for implementing a digital monitoring system for Jersey's older residents.
Rosemarie Finley, the Chief Executive of FNHC, emphasizes the profound impact this will have. "As a community nursing organization, we see first-hand the impact that frailty, falls, poorly managed wounds and long-term conditions like diabetes can have on our patients and their families," she explains. This new system offers their teams a powerful tool to monitor patients more closely without adding extra stress to either patients or staff. The goal? To catch early warning signs, intervene quickly, and help more islanders stay independent, safe, and healthy for longer.
Amy Taylor, Chief Operating Officer at Digital Jersey, highlights the project's potential. "The CareTech Challenge was created to back practical, scalable digital solutions that improve life for older islanders," she states. By collaborating with FNHC and Graphnet, they're introducing proven remote monitoring technology to Jersey's unique healthcare system. And this is the part most people miss: if the pilot program proves successful, there's a real possibility of expanding this model to cover more pathways, settings, and a larger portion of the population in the years ahead.
